They get a complimentary screening (completely free!)

You can fill out the form to give the information of your friend, family, colleague or anyone who deserve high quality care.

What you can win?

• To show our appreciation for your help, your name will be entered into our monthly raffle and you will receive an exclusive 10% off on your next appoitment

• If you refer TWO people for their free screening we will donate $15 to a charity of your choice or pick from our list AND enter another ticket into the rafle

• If you refer THREE people for the free screening we will donate $15 to a charity of your choice or pick from our list AND enter another ticket into the rafle AND you will receive a special gift.

• For all future people you will refer, we will donate $15 to a charity of your choice or pick from our list AND enter another ticket into the rafle.

There is no limit on the number of donation and rafle entries you can enter.

The raffle will be held on the last day of the month and includes gift certifcate for fine dining, gift certifcate for spa services and much more.
